Costs and Fees

The standard state fee for initial registration and renewal fee for both the qualifying patient and caregiver is $100. The fee for replacing a card is $90. However, reduced rates of $25 for initial and/or renewal application fee and $20 for card replacement fee are applicable for both qualifying patients and caregivers who have the following:

• Supplemental Security Income (SSI)
• Medicaid
• Income that does not exceed 200% of the federal poverty level
• Temporary Assistance for Needy Families (TANF)
• State Child Health Insurance Program (SCHIP)
• Housing Assistance
